Assessment User Request Form

To request access to Cambium, DMAC, TestHound, or the Google Drive Campus Testing Confidential folders please complete the Assessment Department User Request Form, available below. A  form is required for new user access and to change user access. Once all signatures are attained, email the completed form to [email protected]. Once your login credentials have been created, you will then receive an email with your credentials. If you have any questions please contact the Assessment Department.
